CAREGIVER

Job Overview:

Caregivers in our community ensure and promote ongoing social and physical stimulation, spiritual, cultural and intellectual needs of the assisted living residents. They assist with supervision and maintaining resident safety especially during periods of heaviest care needs. In addition, caregivers provide direct personal care, enhance resident dining experience, and help maintain clean environment. Caregivers perform and participate in daily activities and events for all residents while promoting resident well-being and satisfaction through support with all activities of daily living.

Resident Care:

  • Provide assistance as required for residents’ activities of daily living. Records care given daily.
  • Ability to assess and to respond to resident and staff medical emergencies following protocol & basic first aid training.
  • Report resident changes to LVN Supervisor or delegate and documents same.
  • Ability to transfer residents with appropriate assistance. (i.e., from bed to wheelchair or from wheelchair to toilet).
  • Lead, participate, encourage and monitor resident participation in social activities.
  • Report changes in resident behavior, mental and physical condition.
  • Follow safety guidelines in the facility, including proper lifting techniques & universal precautions when providing care to residents.
  • Assures that residents’ personal clothing is clean and in good repair.
  • Ability to organize care and services to the assigned group of residents achieving the time lines for attendance at meal times and activities as desired to attend by the Resident.
  • Maintain confidentiality and respect for resident’s privacy. Complies with residents’ rights.
  • Assist with the training and orientation of new staff as it pertains to the role of the caregiver.
  • Clean up following resident accidents, such as diarrhea, vomiting, etc.
  • Gives daily report to oncoming shift personnel.
  • Follows instructions given by Director and/or supervisor.
  • Other duties as assigned.

Preferred Experience:

  • Six (6) months experience working in Assisted or Alzheimer program preferred.
  • Knowledge of medical terminology and proper transfer techniques preferred.
  • General knowledge of oxygen tanks and equipment preferred.

Job Requirements:

  • Current first aid certificate required OR ability to get after 30 days of hire
  • Ability to work with geriatric population, medical professionals, families and staff
  • Fluent English speaking, reading and writing skills
  • Ability to read and interpret written procedures and directions.
  • Ability to pass pre-employment physical/TB test and obtain fingerprint clearance as required by our licensure
  • Ability to work most holidays.

DRIVER

Job Overview:

Our drivers ensure the safe transportation of individuals and groups of Heritage Pointe residents to and from various destinations.

Compensation:

Full-Time Schedule:

Essential Job Duties:

  • Provide transportation in facility vehicles for Heritage Pointe residents.
  • Follow safety rules, ensure the vehicle’s cleanliness, and assist residents with entering and exiting facility vehicles.
  • Assist residents by shopping for items they request and pick up items for other departments.
  • Must be able to lift of move, unassisted, up to 50 lbs.
  • Maintain documentation of travel logs and billing information.
  • Other duties as assigned.

Job Requirements:

  • Possess current California Driver’s License with a Class B requirement which includes the ability to drive a vehicle weighing 34k pounds that is equipped with airbrakes. Passenger Endorsement required.
  • Ability to show clean driving record through DMV and be willing to be included in random drug testing.
  • Ability to pass pre-employment physical/TB test and obtain fingerprint clearance as required by our licensure

Desired Experience:

Job Overview:

Med Techs in our community coordinate resident care related to medications by working with all departments, families and administrative staff to provide for resident needs with continuity and an adherence to the scope of practice and licensure of Heritage Pointe. Provides complete supervision and provision of care related to medication in the community.

MEDICATION MANAGEMENT

  • Administers medications to Resident’s within the scope of practice, adhering to the community’s clinical policy, under the guidance of LVN Supervisor and licensure Heritage Pointe.
  • Communicates Resident status changes to LVN Supervisor(s), physician(s) and other HealthCare providers, as needed.
  • Ensure all medication documentation is current and correct, including medication administration forms, physician orders, change of dosages, written orders, and any others as assigned by supervisor(s).
  • Ensure medication room is completely stocked with all required continuous, replacement of expired, PRN, Over-the-counter, and all other medications as ordered by the physician.
  • Coordinates medication orders and deliveries with pharmacies, including processing new medication orders, refill orders, and following up as needed for medications with prior LVN Supervisor review and approval.
  • Ensures all clinical policies are adhered to in order to maintain the safety of all Residents and adherence to state licensure.

SUPERVISION

  • Assists in the supervision of and provides guidance to CNAs & caregivers when Director and /or LVN Supervisor is not present or working.

RESIDENT CARE

  • Ability to assess and to respond to Resident issues and use proper judgement for same. Follow-up with appropriate paper documentation and advise Program Manager.
  • Able to assist Residents in all ADL needs (Activities of daily living)
  • Coordinate with LVN Supervisor, the care of Residents. Use documentation system to record physical or mental health and reporting changes.
  • Report to LVN Supervisors any medication irregularities.
  • Other duties as assigned.

Job Overview:

We are looking for a highly communicative individual who can warmly greet and assist guests, residents, family members, and visitors at our facility. Our receptionists answer and route telephone calls, provide general administrative support, and handle emergency situations as they arise.

Essential Job Duties:

  • Responds to the community at large as they enter the lobby. Communicates in a pleasant, friendly, helpful manner
  • Answer and route phone calls and relay accurate information over the two-way radio
  • Monitors Philips Med-Alert system, contact healthcare as needed, and run reports
  • Responds and follows through on all procedures and processes for emergencies which includes calling 911, providing Resident Information sheet, and directing personnel as necessary
  • Ensures that all Residents, family members, vendors and visitors sign in and out and are issued a badge or other identifier as needed
  • Schedules and assist with transportation calls and dispatches to drivers
  • Takes and processes housekeeping and maintenance work orders
  • Obtains and records the Daily Census and Resident Roster
  • Receives mail and packages, including medications, and logs and distributes to appropriate personnel
  • Maintains and operates all machines at the front desk. Assists in copying, scanning, faxing, and mailing
  • Keeps all logs and records, binders, and lists up to date including Resident files
  • Maintains a high level or confidentiality regarding Residents and staff of facility
  • Reports and logs any unusual or threatening behavior by a visitor, Resident or employee
  • Completes special projects/assignments and able to provide administrative/clerical support to Administration as needed.
  • Able to be a flexible team member and demonstrate a positive attitude and ability to work well with everyone
  • Other duties as assigned
