Heritage Pointe offers a wide range of personal healthcare services specifically designed to meet the individual needs of our residents. From temporary a la carte services, to our extensive assisted living program, all our healthcare services are designed to promote personal wellness and allow as much independence as a resident’s capabilities permit.
Services Available To All Residents:
- Ongoing monitoring and evaluation by the healthcare services department to determine appropriate level of care
- Individual conferences with healthcare services director or administrator, available by appointment
- Needs and services assessment plans performed upon move-in and updated regularly by Heritage Pointe interdisciplinary team
- Ongoing psychosocial intervention
- Consultation with staff and other professionals
Heritage Pointe will continue to monitor your needs and periodically reassess the services being provided to you. Assessments may result in changes in the services provided to meet your needs and the assisted living fee. A change in your level of care will be determined by Heritage Pointe’s interdisciplinary team of professionals and does not require a 30-day notice.
Extensive services are available at additional cost.
